Am I miss understanding how CKS:FBA works, I thought it was going to replace my login page so I could customize it and make it personal to each site I have FBA on. Is this not the case?

I'm trying to get rid of the bland login page with form auth.

No CKS FBA does not replace the original out of the box FBA login form if the whole website was entirely secured and only authorised logon users can view the entire content. So you will have to manually customise the FBA login form like the one I did at that uses CKS FBA

If CKS FBA was used on a public internet site where users can view the site but only some pages or subsites are secure to only authorised login users who can view the secure content, then you use the CKS FBA web part like in the this site for example

Ahh yes your first link is what I was trying to figure out how to do. Sadly I'm no ASP/C# expert and I was trying to poke around the _layout folder and it is convoluded as all get out. Is there any sites that give a dumb down understanding on how to do that?
